CONSULTATION: CONSENT FORM
A good consultation can set you up for a smooth running appointment and a good consent form can set you up for running smoothly after the appointment. Plan for a few minutes at the beginning of each new client appointment to review the consent form and have them sign. This is an important part of the consultation! As a precaution, review your consent form with your insurance and lawyer. Here’s a list of items to consider including on your consent form.
Basic Information
Ask a few basic information questions: name, phone number, email, birthdate. It’s good to have contact information on hand and also a way of communicating any changes or announcements. You could also go the extra step of asking for an emergency contact in the case of an emergency happening while they are at their appointment, and If you keep track of client referrals have them write down who referred them.
Overview
It might seem obvious why the client is there, but it’s a good idea to have an overview of the appointment explaining the service and what is expected of them. Such as keeping their eyes closed, how long the appointment is, and the agreed price.
Allergies
Include a section for them to sign and agree to disclose any allergies they may have relating to latex, surgical tapes, cyanoacrylate, etc. There can be a section that lets them know some medications can affect their retention.
After Care
It’s a good idea to include an overview of your after care so that the clients have a chance to read it themselves. It is more likely they will follow proper aftercare procedures if they are signing to agree.
Photos
A portion of your consent form should be the client giving permission to use photos of them to show to other clients and posting online. It’s better to have the client agree and sign so there is no discrepancy later on.
